Ages and Stages Questionnaire for Kindergarten Enrollment 

We're excited to have your child in kindergarten this year! The first five years of a child's life are super important for their growth and learning. That's why we're using a special tool called Ages and Stages, ASQ, to learn more about each child and to give personalized instruction in kindergarten. The tool helps us see how kids are doing in different areas like talking, moving, social skills, problem-solving, and behavior. It tells us what you already know about your child.  It's not about right or wrong answers. We want to know what your child can do now and where they might need some help. It's easy and quick, only taking about 10-15 minutes. 

Here's how ASQ-3 works: 

  1. Complete the questions when your child is with you, so you can ask them if needed. 
  2. Each question has three options: "yes," "sometimes," or "not yet." You choose what fits your child best. Your answers help us know their strengths and areas where they may need practice. 
  3. You can try fun activities with your child to help answer the questions. It's all about playing, moving, and doing regular things. 
  4. After you finish the questions, a professional will share the results with you. 

Here's how ASQ-SE2 works: 

  1. You can do these questions without your child being there. 
  2. For each question, choose "often or always," "sometimes," or "rarely or never" based on what you know about your child. 

  3. If you're worried about any behaviors, make a note of them. 

  4. Remember, your answers help us see your child's strengths and where they might need support. 

Please select the school you are enrolling your child into for kindergarten to complete the questionnaire. If you are a transfer to a school and have completed the transfer paperwork, select the school you are transferring to for your kindergarten year. If you have any questions, please call your school's office.
