AGE
REQUIREMENTS
All students entering Kindergarten must by five (5) years
old on or before August 31, 2008. First grade students must be six
(6) years old on or before August 31, 2008 – or have attended an
accredited Kindergarten, according to Kansas Statute K.S.A. 72-1107 which
defines the entrance requirements for the state.
NEW STUDENTS TO THE DISTRICT
Students new to Geary County Schools USD 475 MUST provide
the following:

CERTIFICATE of IMMUNIZATION CARD
2.
REQUIRED
IMMUNIZATIONS
3.
CHILD HEALTH ASSESSMENT
(physical examination for children eight years-old and
younger)
4.
PROOF OF
IDENTITY
— Birth
Certificate (required for Kdg.); Current passport
or driver’s license.
5. GRADE CARDS, CERTIFIED TRANSCRIPTS
6.
KSHSAA PHYSICAL EXAMINATION
(7th – 12th grade students)
