REQUIRED
IMMUNIZATIONS
Kansas
law (K.S.A. 72-5209)
requires the following immunizations in order to attend schools in
Kansas.
Required for School
Attendance in Gr. 6-12:
-- DPT, DTaP and/or DT - 5 doses (4, if the 4th does is
after the 4th birthday)
-- Td/Tdap booster is recommended at 11-12
years of age but required 10 years after the last
DTaP,
usually middle school or high school age
-- Polio - 4 doses (3 if the 3rd doses is after the 4th
birthday)
-- MMR - 2 doses (the 1st must be after the 1st
birthday)
(Meningitis vaccine is recommended for middle and high school
students)
Required for School Attendance in Gr. K-5:
Grades K-5 must meet all the requirement for Grades 6-12
and the following:
-- Hepatitis B series .....3 doses (last dose must be
after 24 weeks of age)
-- Varicella (Chickenpox) - 1 does required, unless child has
had chickenpox
2 doses are recommended for all students
Required for School Attendance for Pre-K
(ages 3-4)
-- DPT, DTaP and/or DT - 4 doses
-- Polio - 3 doses
-- MMR - 1 (after 1st birthday)
-- Hepatitis B series and Varicella (Chickenpox)
School Nurses will review all immunization records
for other requirements (spacing of shots)
