DIRECTIONS    How to download and use forms

• Click on one of the forms above to download.

• When the "File Download" window appears, choose

• Choose to save the file in your home folder on the network.   

• Locate the file where you just saved it and double-click it to open it. You must have Microsoft Excel 97 or greater to use the form

• Fill in the document as you would a normal form

• Save, print BOTH SIDES, and close the document when completed

• Get appropriate signatures on the printed form and send to Christy Gonalez, Devin Center. Be sure to send BOTH SIDES to Christy.
